The following services are offered under the J Choisius LLC umbrella and through affiliated licenses. Each service line operates under its own brand and applicable licensing:

2. Placement Services — Free to Families

ChoicePath Care Placement, operated by J Choisius LLC, provides assisted living placement consulting at no cost to families or individuals. Our placement service is made possible through referral compensation paid by assisted living communities in our network upon a successful placement.

• We do not guarantee placement in any specific community
• All recommendations are based on information provided by the family and our knowledge of available communities
• Final placement decisions remain solely with the family and the chosen community
• We do not provide medical advice or diagnoses
• We do not act as a licensed healthcare provider

3. Real Estate Services

Real estate services are provided by Jacqueson Choisius, licensed REALTOR® with LoKation Real Estate, a Florida-licensed real estate brokerage operating independently from J Choisius LLC. All real estate transactions are conducted under Jacqueson Choisius @ LoKation Real Estate and are subject to:

• Florida real estate laws and regulations
• The Fair Housing Act — we do not discriminate on the basis of race, color, national origin, religion, sex, familial status, disability, or any other protected class in the provision of our real estate services
• Applicable buyer, seller, and landlord-tenant agreements
• MLS rules and regulations where applicable

Real estate commissions and fees are disclosed separately in applicable listing or buyer representation agreements.

4. Insurance Services

Insurance guidance provided by Jacqueson Choisius is for informational and advisory purposes only. We do not bind coverage or issue policies directly. All insurance products are subject to carrier terms, conditions, and underwriting approval. Clients are encouraged to review all policy documents carefully before making coverage decisions.

5. Event & Photography Services

360 photo booth and event photography services provided through JC360 Images, operated by J Choisius LLC, are subject to separate service agreements, booking deposits, and cancellation policies communicated at the time of booking. J Choisius LLC retains the right to use event photos for portfolio and marketing purposes unless otherwise agreed in writing prior to the event.

6. No Medical or Legal Advice

Nothing on our website or in our consultations constitutes medical, legal, or financial advice. All information provided is for general guidance purposes only. Families are encouraged to consult with licensed medical professionals, elder law attorneys, and financial advisors when making senior care decisions.

7. Fair Housing Compliance

Jacqueson Choisius and J Choisius LLC are fully committed to compliance with the Fair Housing Act and all applicable Florida fair housing laws across all service lines. We provide equal service to all clients and families regardless of race, color, national origin, religion, sex, familial status, disability, age, marital status, or any other protected characteristic.

• Decisions made by families based on our placement recommendations

• Real estate market conditions or investment outcomes
• Insurance carrier decisions or coverage denials

12. Governing Law

These Terms and Conditions are governed by the laws of the State of Florida. Any disputes arising from these terms or our services shall be resolved in the courts of Broward County, Florida.

J Choisius LLC reserves the right to update these Terms and Conditions at any time. Changes will be posted on this page with an updated effective date. Continued use of our website or services after changes constitutes acceptance of the updated terms.
